I like the Grim Reaper's Spirit perk. You can destroy everything in your path with this perk. That is, if you have a great gun.

Mysterious Stranger and Bloody Mess are really great and all, but my favorite has to be the Cannibalism perk. I mean cmon, what other video game can you eat other people. I mean it's just like the way I eat--AH BLAST!!! I'VE REVEALED MYSELF AGAIN!!

Almost Perfect is a very good perk since it makes your character noticeably more powerful. Mister Sandman is entertaining but hard to use frequently since you have to catch npc's sleeping.
However, the perk I must have in every playthrough is the Swift Learner perk.

Been too long away from the wastes to remember all my faves (some are really cool, others are just handy). Like Animal Friend is handy, even if you just put enough points in it so that animals (read Yao Guai) leave you alone. Bloody Mess is pretty cool I guess, but I actually prefer to see the varied death animations instead of always just the blowing up with body parts everywhere. With Bloody, you miss out on some of the more hilarious deaths that the physics engine makes possible (although probably not strictly intentionally). I always get Comprehension before I read any of the books, so that I can get the max possible skill points from the books. Black Widow/Lady Killer are both pretty cool for the added conversation options, and both are handy for fighting raiders (as there are at least as many female raiders as males, though many of the other human enemies are mostly males). Sneaking is pretty handy, so I usually try to get Silent Running. I tend to use a lot of chems to survive the wasteland (usually have my difficulty set several clicks up), so Chem Resistant and Chemist are both handy.
Really between all my different characters, there's only a few I don't have, and several I have on pretty much any character (like comprehension for example, and usually Strong Back, which I still tend to think of as Pack Mule).

Grim Reapers Sprint is my favorite. Although it makes the game super easy, I find the most helpful when im playing at higher difficulties.

I love animal friend, but I wish it worked for radscorpions! I HATE those *** things. I also like cannibal, grim reaper's sprint, and especially strong back and solar powered.
