One of my absolute favorites was the time my brother and I were playing the campaign in Star Wars: Battlefront II. We were stuck on Polis Massa. You know, the asteroid? It's a tough level because the enemies are unlimited and kill you fast unless you complete each objective.
So after losing about 5 or 6 times in a row, we found ourselves holed up in a base on the opposite side from the enemy spawn point, relying on the ammo and health droids to stay alive. They were coming in little waves, which we took down one at a time in the hallways.
After 20 minutes of this, we got tired of arguing about which base to go for. We were the last two clones in the whole place, so if either of us died, it was game over. Naturally, we decided to rush the spawn point since we had nothing to lose.
We ran across the entire base and took almost no damage because while we were hiding out, we had gotten every medal at least twice for the kills we made just sitting there, making us effectively invincible. We got to the objective in a room full of about 30 rebels, and mowed them down faster than they could respawn with our upgraded weapons. Took out the objective, and the next thing we knew it was over.
Then the end of level screen came up. We both fell off the couch, hysterical, because we ended up killing about 1000 rebels in one battle from our little base-camp. It was unbelievable.
Not sure this is very exciting, but the most memorable co-op moment was when me and my sister beat Resident Evil 5 on all of the difficulties including professional. Also, we both got all of the original achievements as well.
Mmm... Favorite moment in Co-Op gaming.... Playing through all of Perfect Dark on the N64, mowing down all those enemies together and that final boss... That was entertaining =]
Most memorable moment in Co-Op would be taking down General RAAM in Gears of War with my brother on Insane. He got his ass nailed pretty quick, but I picked him up, and torqued away. I had to shift sides and he got nailed in the midway. No time to run back, I lancered him until my health was almost full in the red. THEN! He died, there was much cheering and rejoicing as the cut scene began.

When, playing through Army of 2 on Hard, Me and my cousin fought through the obscenely hard China map, and at the very end he turned to me and said "Well, there goes the Chinese Army".
Second: Also on the China map, where you have to blow up the tank under the bridge. We co-op sniped it. "3...2...1..." we shot, and nothing happened. "Huh?" he said. I turned to him and realized that we both forgot he was holding the rocket launcher. Oops. Got the job done, just took an extra ten seconds.
Every moment of Resident Evil 5. It was spectacular. Not to mention you didnt have the retarded computer muffing up crap.
Halo 3 till midnight on matchmaking with 5 people, switching off on every death. Funny and entertaning :)

The best co-op experience I have ever had was when I invited 5 friends over to my house to play Call of Duty 4. But what made it great was that all my friends brought over their Xbox 360's and a TV each. We hooked it all up and then played together, in a party, on Xbox Live, while in the same room. That was the best idea I have ever come up with and my best co-op experience!
Favorite Co-op experience, huh? Hm.... either the L4D2 Demo(all of it), or one match in Uncharted 2's Co-op where we were coordinated perfectly, and none of us were using our mics.

I don't have any awesome, specific story, but running through Perfect Dark, especially the challenges, was a blast.
Champions of Norrath (and Return to Arms) on PS2. Splitscreen coop with my lady. Such an expertly crafted console action RPG/hack and slash... we still haven't seen any this generation.
Best Co-op Moment:
Halo: Combat Evolved. Me and my buddy were riding around in the warthog, I had a rocket launcher and was riding shotgun. I aimed down, shot, and flipped the warthog over a rock. This rock was supposed to stop us from taking the warthog any further, but since I accidentally got us past it, we could bring it all the way in the building. This incited us to try and get the warthogs up to stupid places and just do stupid crap in the warthogs for the next few weeks, it was pretty fun. :D
When I die, I want to go peacefully like my Grandfather did, in his sleep -- not screaming, like the passengers in his car.
