It's hard to get my head around the idea of an epic, open world John Carmack game. In my memory, most of his work has been more of the claustrophobic, linear atmospheric storytelling variety. Still, if reviews are to be believed, this might be a must play title. It does remind me rather too strongly of Fallout 3 and Borderlands though so I hope playing it doesn't come across as redundant.
