I've been fairly lucky, but I'd have to say LOTR: Conquest.
My top 5 worst games...
1 - ET(Atarti 2600), its horrible. It's poorly designed, and completely impossible to know what to do without the manual. You'll spend alot of time falling in and out of holes.(sometimes right back into the same hole you just left.)
2. Megaman X7(PS2), my goodness this game crapped all over the X series. I didn't intially like X 6 all that much(it's a so so rush job to make it out before the PS1's deaththrows.), but I quickly forgave any of it's flaws after playing X 7. The controls are so awkward, it makes what used to be an easy to play game, into a nightmare. The graphics are ugly, and the voice over work sounds like the janotiral staff did it at capcom. I'm a diehard Megaman fan, and I couldn't force myself to finish it.(I have beat every single game of the regular series, and all X series including command mission besides it.) My friend and I always joke one day we will actually sit down and play thru it entirely, misery loves company. I almost didn't play X 8 because of it.
3. Street Fighter:the movie(PS1), lets make a bad movie after an awesome game series, then make an awful game after said movie. It was the video game version of multiplicity, where each clone gets progressly worse. Using love actors and motion capturing them doing special attacks(just like the original Mortal Kombat, as the special attacks look like rodger rabbit next to the live actors in terms of realism next to each other.) with lots of missing frames of animation is always wonderful.
4. (NES/SNES/GENESIS)16 and 8 bit era movie/show time ins. There was so many bad ones, I will toss them all on the same bus and push them off a cliff hoping never see them again. (movie ties suck majority of the time even today.) I thankfully didn't own many of them as I knew better. But people I knew would rent them. (particularly my cousin as his mom had no idea what games were good or not.)
5. Deadly towers(NES). The game was pure madness. I kenw some many poor kids back in the 80s who bought for it's cool box art, and were subjected to this piece of crap.
I actually have never played a bad game. My only issue was with the online people, not the game.
Legends of Wrestling for GameCube. I'm not a huge fan of wrestling games to begin with, but I'm willing to at least give them a chance (I have nothing against the genre, they're just not my type of game). This one was terrible. I think the unresponsive controls were probably the worst thing about it. I said PUNCH, YOU IDIOT. Thankfully, the only reason we had it was because it came as a set in an eBay purchase of Pikmin, so we didn't feel too bad chucking it.

The worst game I have ever played definitely had to be NBA Ballers: Chosen One. Worst. Ten. Dollar. Rental. Ever.
I'm so glad I only rented it.
I thought it had a great story, but the game play could have been much much better.
