iv'e been wondering the same thing.
I like to start on hard.
I play shooters on extreme, fighters on very easy, and everything else on regular.
I like to play on the default the first time through. After that i try and beat it on the hardest difficulty.

I play on the hardest setting. Not because i think im good or anything but, if im buying a game (especially a FPS), i want to feel like my money was well (?) spent. Yes, at times, i get really frustrated and, at times, have changed the difficulty settings (especially when you die due to a glitch against a hard boss, not actual gameplay).
Also, there are achievements that can only be achieved through the hardest setting. Unless its just a rental, i dont try to rush through a game.
Hardest, to beat the game on the hardest difficulty is an accomplishment in itself. It makes it feel more complete to me.
Ususally I'll do the default, then keep going upwards in difficulty.

It depends on the game. I'll usually play through the first time on hard or default setting, then play again on the hardest difficulty if I like the game enough and if the game is co-op like Halo or Gears of War.
